Nota: C. longa wordt voornamelijk ook gebruikt als specerij. Gedroogde en gemalen wortel van de

geeft een intense, warme gele kleur aan gerechten en is ook verantwoordelijk voor het geel in currypoeder. De smaak is quasi afwezig en het aroma eerder zwak. De gele kleur komt van de stof curcumine, die slecht oplosbaar is in water. De kleur is afhankelijk van de zuurtegraad. In Indonesië noemt men kurkuma koenjit. De EEG kent deze natuurlijke kleurstof als voedingsadditief E100, dat veel gebruikt wordt in snoep, gebak en pudding.

Geelwortel (Curcuma longa) Geelwortel of kurkuma behoort tot de gemberfamilie (Zingiberaceae). Het is een 0,6-1,5 meter hoge plant met korte stengels, langwerpige, gepunte bladeren en gele trechter-vormige bloemen die bij elkaar staan in de vorm van een bloeikolf. De primaire wortelstok is rond tot ovaal met hieraan 4-7 cm lange en 1-1,5 cm dikke secundaire wortelstokken [1,2,3]. C. longa wordt op grote schaal verbouwd in India, China en andere tropische regio’s en de gedroogde gemalen wortel wordt om zijn aromatische geur en kleur veel als specerij gebruikt, vooral als ingrediënt in curry’s [1,3]. Zoals de naam al aangeeft, heeft het wortelpoeder een sterk gele kleur, een kenmerk dat ook tot uitdrukking komt in de Sanskriet naam van geelwortel: haridra (hari = geel, dra = rennen of haast maken; vanwege de snelheid waarmee textiel gekleurd wordt) [2]. Kenmerkende inhoudsstoffen in de geelwortel zijn met name curcumin en curcuminderivaten die tot 0,3-5,4% van het gewicht van de wortel kunnen uitmaken. De wortel bevat ook substantiële hoeveelheden vluchtige olie met α- en -tumeron en zingiberon als karakteristieke bestanddelen [1,2,3,5]. Traditionele (volks)geneeskundige toepassingen van geelwortel, zowel binnen de ayurveda als de traditionele Chinese geneeskunde, hebben vooral betrekking op de ontstekingsremmende, digestieve en hepatoprotectieve activiteit alsmede op de behandeling van onder andere koliek, winderigheid, bloedingen en menstruele problemen [1,3,5]. Talloze laboratoriumexperimenten en dierproeven hebben ook laten zien dat geelwortel en/ of het hieruit geïsoleerde curcumin een antiparasitaire, spasmolytische, hepatoprotectieve en choleretische, ontstekingsmodulerende, wondhelende, antioxidatieve en chemopreventieve werking hebben [3,5,6,7,8,9]. Met name in relatie tot het voorkomen en behandelen van kanker is het mogelijke nut van curcumin de laatste jaren eveneens belicht (zie bijvoorbeeld [6,8,9]); curcumin blijkt namelijk diverse processen waaronder carcinogenese, signaal-transductieroutes in tumorcellen, geprogrammeerde celdood (apoptosis) en angiogenese zodanig te beïnvloeden dat een positief effect bij kanker verondersteld kan worden [8].

Wetenschappelijke publicaties met betrekking tot klinisch onderzoek aan geelwortel als monopreparaat zijn momenteel echter nog slechts in beperkte mate beschikbaar. De uitkomsten van drie klinische studies [10,11,12] tonen aan dat het effect van geelwortel op spijsverteringsklachten (dyspepsie, gastritis en irritable bowel syndrome) wisselend is, waarbij een hoge dosering (3 g geelwortel per dag) mogelijk pijn en ongerief reduceert [12]. Bij deze dosering lijkt geelwortel ook een anti-ulcerogene activiteit te bezitten [12]. Bij patiënten met aandoeningen aan de luchtwegen is waargenomen dat geelwortel verlichting geeft van symptomen zoals kortademigheid, hoesten en slijmvorming [3]. In een studie van geringe omvang werd ook een duidelijke verbetering van reumatische klachten gemeld na orale toediening van 120 mg geelwortel per dag [3]. Een preparaat waarin C. longa wordt gecombineerd met onder andere winterkers laat bij patiënten met osteoartritis ook een significant positief effect zien op de hoeveelheid pijn en de mate van invaliditeit hoewel radiologisch onderzoek geen veranderingen laat zien [13].

Geelwortel anti-catabool, tegen spierafbraak

Geelwortel is een specerij uit India, en heet ook wel koenjit of kurkuma. De specerij wordt gemaakt uit de gedroogde wortels van de plant Curcuma longa, een verwant van de gemberplant Zingiber officinale. Aan kleurstoffen uit C. longa dankt saffraanrijst zijn gele kleur. Geelwortelpoeder is een bestanddeel van kerrie.

Traditionele Indiase genezers maken al eeuwen gebruik van de ontstekingsremmende eigenschappen van geelwortel, en geven de plant aan mensen met ademhalingsproblemen of wonden die moeizaam genezen. Een belangrijke actieve stof in de plant is curcumin of diferuloylmethaan. Dat is overigens op de markt als gele kleurstof voor voedingsmiddelen. Je ziet de chemische structuur van curcumin hieronder.

Sommige onderzoekers vermoeden dat curcumin de hersenen beschermt. In dierproeven vermindert curcumin de schade die cadmium en lood aanrichten in de hersenen. Epidemiologen vermoeden dat curcumin de hersenen beschermt tegen de ziekte Alzheimer, en dat daarom de ziekte in India - waar de consumptie van koenjit hoog is - minder vaak voorkomt dan op andere plekken in de wereld.

Ook de anti-catabole werking van geelwortel is het werk van curcumin, blijkt uit een overzichtsartikel van onderzoekers van Harvard Medical School. Dat gaat vooral over curcumin als medicijn tegen spierafbraak door bloedvergiftiging, de specialisatie van de onderzoekers. Maar, schrijven ze, "there is evidence that curcumin may inhibit the catabolic response in skeletal muscle during other catabolic conditions".

Ze doelen daarbij op dierstudies die het anti-catabole effect van curcumin hebben aangetoond bij kanker en beschadiging van het spierweefsel. In sommige van die studies versnelde curcumin ook het herstel van het spierweefsel.

In studies waarin proefdieren hun spieren niet konden gebruiken remde curcumin de afbraak van spiereiwit trouwens niet. Curcumin werkt dus niet altijd.

Curcumin, vermoeden de onderzoekers, stimuleert de spieropbouw vooral via het saboteren van de moleculaire schakelaar [eigenlijk: transcriptiefactor]nuclear factor-kappa-B (NFkB). NFkB reageert op zo ongeveer wat slecht is voor de cel: van vrije radicalen tot UV-licht tot geoxideerd LDL-cholesterol. De transcriptiefactor maakt zich in zulke gevallen vast aan het DNA, en schakelt zo een batterij vernietigingsmechanismen aan.

Bij bloedvergiftiging, maar ook bij kanker, zware ingrepen en ondervoeding ontstaat via NFkB soms grote schade aan het spierweefsel. Curcumin is goedkoop en ook in hoge doses niet toxisch. Het is misschien een alternatief voor anabole hormonen, die prijzig zijn en soms bijwerkingen hebben. Vandaar dat de onderzoekers er graag meer over willen leren.

Een probleem is dat het lichaam curcumin slecht opneemt. Bedrijven werken daarom aan nano-preparaten van curcumin. Een alternatief is curcumin toe te dienen met peper, aldus de onderzoekers. "Another method to increase the bioavailability of curcumin is to add piperine, found in black pepper, which increases the uptake of curcumin by 2000 per cent in humans."

Voor de goede orde: nee, de schrijvers van het stuk zijn niet van plan zelf curcumin als medicijn op de markt te brengen. Ze worden betaald door het Amerikaanse equivalent van ons RIVM, de National Institutes of Health.

Hoeveel spieren je ontwikkelt is de resultante van je anabole en katabole processen. Verhoog je het anabolisme in je lichaam, dan ga je sneller vooruit. Verminder je het katabolisme, dan ook.

Of curcumin ook antikatabool werkt tijdens en na zware training is nog niet onderzocht.

The medicinal properties of curcumin obtained from Curcuma longa L. cannot be utilised because of poor bioavailability due to its rapid metabolism in the liver and intestinal wall. In this study, the effect of combining piperine, a known inhibitor of hepatic and intestinal glucuronidation, was evaluated on the bioavailability of curcumin in rats and healthy human volunteers. When curcumin was given alone, in the dose 2 g/kg to rats, moderate serum concentrations were achieved over a period of 4 h. Concomitant administration of piperine 20 mg/kg increased the serum concentration of curcumin for a short period of 1-2 h post drug. Time to maximum was significantly increased (P < 0.02) while elimination half life and clearance significantly decreased (P < 0.02), and the bioavailability was increased by 154%. On the other hand in humans after a dose of 2 g curcumin alone, serum levels were either undetectable or very low. Concomitant administration of piperine 20 mg produced much higher concentrations from 0.25 to 1 h post drug (P < 0.01 at 0.25 and 0.5 h; P < 0.001 at 1 h), the increase in bioavailability was 2000%. The study shows that in the dosages used, piperine enhances the serum concentration, extent of absorption and bioavailability of curcumin in both rats and humans with no adverse effects.

AIMS:

Turmeric extract derived curcuminoids (curcumin, demethoxycurcumin and bisdemethoxycurcumin) are currently being evaluated for the treatment of cancer and Alzheimer's dementia. Previous in vitro studies indicate that curcuminoids and piperine (a black pepper derivative that enhances curcuminoid bioavailability) could inhibit human CYP3A, CYP2C9, UGT and SULT dependent drug metabolism. The aim of this study was to determine whether a commercially available curcuminoid/piperine extract alters the pharmacokinetic disposition of probe drugs for these enzymes in human volunteers.

METHODS:

A randomized placebo-controlled six way crossover study was conducted in eight healthy volunteers. A standardized curcuminoid/piperine preparation (4 g curcuminoids plus 24 mg piperine) or matched placebo was given orally four times over 2 days before oral administration of midazolam (CYP3A probe), flurbiprofen (CYP2C9 probe) or paracetamol (acetaminophen) (dual UGT and SULT probe). Plasma and urine concentrations of drugs, metabolites and herbals were measured by HPLC. Subject sedation and electroencephalograph effects were also measured following midazolam dosing.

RESULTS:

Compared with placebo, the curcuminoid/piperine treatment produced no meaningful changes in plasma C(max), AUC, clearance, elimination half-life or metabolite levels of midazolam, flurbiprofen or paracetamol (α = 0.05, paired t-tests). There was also no effect of curcuminoid/piperine treatment on the pharmacodynamics of midazolam. Although curcuminoid and piperine concentrations were readily measured in plasma following glucuronidase/sulfatase treatment, unconjugated concentrations were consistently below the assay thresholds (0.05-0.08 μM and 0.6 μM, respectively).

CONCLUSION:

The results indicate that short term use of this piperine-enhanced curcuminoid preparation is unlikely to result in a clinically significant interaction involving CYP3A, CYP2C9 or the paracetamol conjugation enzymes.
