Prostaat

De prostaat is een kleine klier, ongeveer zo groot als een walnoot, die als een ring net onder de blaas en rond de urinebuis ligt. Een normale prostaat weegt zowat 20g. De prostaat bestaat uit een binnenste gedeelte en een buitenste gedeelte, een beetje zoals een vrucht met een schil. Zoals bekend hebben alleen mannen deze klier. Hij produceert het grootste deel van de zaaduitstorting, namelijk het vocht dat dient als transport en voeding voor de zaadcellen. Rond de prostaat liggen bloedvaten en zenuwen die essentiëel zijn om een erectie te kunnen krijgen.

Goedaardige vergroting van de prostaat (Benigne prostaathypertrofie, BPH)

Op oudere leeftijd, meestal vanaf 50 jaar, kan de prostaatklier groter worden. Dan gaat het plassen moeilijker. De straal wordt minder krachtig. Men noemt dit ook wel de "oude-mannen kwaal". Ook duurt het langer voor het plassen op gang wil komen, terwijl men toch aandrang heeft. Na de plas kan er nog wat urine nadruppelen. Soms moet men `s nachts naar het toilet. Deze klachten ontstaan omdat de urinebuis wat wordt dichtgedrukt door de prostaat. Als de prostaat nog groter wordt, kan er steeds urine in de blaas achterblijven en ontstaat er soms een blaasontsteking.

Wat is prostaatvergroting?

Alleen mannen hebben een prostaat. Het is een klier onder de blaas, zo groot als een kastanje. De plasbuis loopt vanuit de blaas eerst door de prostaat en vervolgens door de penis naar buiten. De prostaat produceert het vocht waar de zaadcellen in zwemmen. Bij een zaadlozing komt dit vocht via de plasbuis naar buiten.

Op oudere leeftijd wordt de prostaat meestal groter. De oorzaak hiervan is onbekend. Deze vergroting is goedaardig en heeft niets met kanker te maken. Wel kan een vergrote prostaat de plasbuis dichtdrukken. Hierdoor krijgt u problemen met plassen. Alcohol en sommige medicijnen kunnen deze klachten verergeren.

Hoe kunt u prostaatvergroting herkennen?

U moet vaker plassen, overdag maar ook `s nachts. Uw plas komt moeilijk op gang of u heeft een zwakkere straal. U kunt last hebben van nadruppelen of van het gevoel dat uw blaas niet leeg is na het plassen. Het kan voorkomen dat u de plas minder goed kunt ophouden of dat u tijdelijk niet kunt plassen.

Wat kunt u zelf doen aan prostaatvergroting

  • Stel het plassen niet uit als u aandrang voelt.

  • Neem de tijd om te plassen.

  • Plas bij voorkeur zittend.

  • Drink niet meer dan twee glazen alcohol per dag.

  • Als u erge aandrang voelt en toch niet kunt plassen, neem dan contact op met uw huisarts.

Welke geneesmiddelen kunnen worden gebruikt bij prostaatvergroting?

Alfablokkers

Alfablokkers ontspannen de spieren van de blaashals en de prostaat. Hierdoor vermindert de vernauwing van de urinebuis en kan de urine beter door de plasbuis stromen. De prostaatvergroting zelf verandert niet, maar klachten als vaak moeten plassen zullen verminderen. Voorbeelden zijn alfuzosine, doxazosine, prazosine, silodosine, tamsulosine en terazosine.

Anti-androgenen

Anti-androgenen verminderen de werking van het mannelijk geslachtshormoon testosteron. Testosteron is verantwoordelijk voor de groei van de prostaatklier. De anti-androgenen remmen de groei van de prostaat en verminderen klachten als vaak moeten plassen. Het duurt echter minstens zes maanden voor u dit effect kunt merken. Daarom schrijft de arts in eerste instantie meestal een alfablokker voor. Daarvan merkt u al effect veel sneller.

Artsen schrijven anti-androgenen alleen voor als de prostaat sterk is vergroot, als de alfablokkers niet voldoende effect hebben en als u niet een operatie kunt of wilt ondergaan. Voorbeelden zijn dutasteride en finasteride.

Interactie: Het voedingssupplement quercetine, dat door ouderen vaak gebruikt wordt als zelfmedicatie bij prostaatproblemen, heeft een negatief effect in combinatie met tamsulosine, een geneesmiddel dat ook bij dergelijke aandoeningen wordt gebruikt. Ouderen zouden hierdoor meer risico lopen om te vallen en bijvoorbeeld hun heup kunnen breken. Dat schrijven onderzoekers van Maastricht Universitair Medisch Centrum in het wetenschappelijk tijdschrift European Journal of Pharmacology. Het onderzoek is uitgevoerd in samenwerking met de Nederlandse Voedsel- en Warenautoriteit (NVWA).

Een goedaardige vergroting van de prostaat is een probleem waar zeventig procent van alle mannen boven de zestig jaar last van heeft. Het medicijn tamsulosine is een zeer effectief middel tegen dergelijke prostaatproblemen. Het geneesmiddel verslapt de spieren van de urinewegen, zodat patiënten beter kunnen plassen. Eén van de bijwerkingen is echter dat ook bloedvaten kunnen ontspannen (relaxeren), waardoor de bloeddruk plotseling kan dalen bij het opstaan. Patiënten kunnen daardoor vallen met mogelijk botbreuken tot gevolg. Vooral onder ouderen kan dat gevaarlijk zijn. Na bijvoorbeeld een heupbreuk overlijdt de helft van de mensen binnen een jaar. Het voedingssupplement quercetine versterkt het valrisico nog eens aanzienlijk, zo blijkt uit het onderzoek.

Quercetine is een stof die in lage hoeveelheid voorkomt in onder andere appels, uien, rode wijn en groene thee. Als voedingssupplement heeft het echter een veel hogere dosis. Het werkt onder andere cholesterolverlagend en ontstekingsremmend. Quercetine is als antioxidant bij de drogist verkrijgbaar. Een belangrijke toepassing is zelfmedicatie bij prostaatproblemen. Volgens de Maastrichtse onderzoekers kan de combinatie van quercetine met het prostaatmedicijn tamsulosine echter gevaarlijk zijn. De combinatie van beiden zorgde ervoor dat het effect van het medicijn op de bloedvaten tot maar liefst dertien keer werd versterkt en dus het risico op vallen aanzienlijk vergroot.

Kruidenpreparaat tegen prostaatkanker

Kwaadaardige prostaatkanker is een ernstige ziekte. Wanneer een soort voorlopercellen van prostaatkanker (HGPIN: hooggradig prostaat intra-epitheliale neoplasie) in een prostaatbiopsie wordt gevonden, dan is de kans op kanker vergroot. Ongeveer 30% tot 60% van de mannen met HGPIN zullen binnen 5 jaar prostaatkanker ontwikkelen. Een van hypotheses aangaande het ontstaan van HGPIN en uiteindelijk prostaatkanker is chronische ontsteking van de prostaat. Ontstekingsremmers zoals NSAID’s, en COX-2 remmers en bepaalde kruiden bleken in laboratorium- en dierstudies een positief effect te hebben op HGPIN en prostaatkankercellen. Echter, humaan onderzoek is nodig om de waarde van deze stoffen te bepalen bij mannen met HGPIN en prostaatkanker.

In de volgende fase I studie is onderzocht of mannen met HGPIN geen ernstige bijwerkingen ondervonden bij het gebruik van een bepaald kruidenpreparaat (Zyflamend) en voedingssupplementen gedurende 18 maanden. De onderzoekers hebben de studie zo opgezet om zo goed mogelijk de alledaagse situatie te simuleren. De eerste 3 mannen kregen 3 maal daags 780 mg Zyflamend capules. Wanneer Zyflamend na 1 maand goed verdragen zou worden, dan zouden de volgende 3 mannen naast Zyflamend 1 capsule probioticum per dag krijgen. Bij het goed verdragen van de capsules kregen de volgende 3 mannen steeds weer 1 capsule extra van het volgende supplement.

Zyflamend bestaat uit o.a. uit rozemarijn, geelwortel, gember, groene thee..... zie verder juiste samenstelling. Gedurende ruim 1 jaar werden 29 mannen met HGPIN gerecruteerd voor de studie. Zes mannen hebben de studie van 18 maanden niet afgerond vanwege persoonlijke redenen. Alle groepen konden Zyflamend en de andere supplementen goed verdragen en ondervonden geen ernstige bijwerkingen. Ongeveer 40% van de mannen had last van licht maagzuur dat snel verdween na iets eten. Een patiënt had een paar dagen in de eerste 2 maanden last van diarree.

Ook werd in de studie gekeken naar een prostaattumormarker (PSA). Na 18 maanden bleek dat bij een groot gedeelte het gehalte van het PSA duidelijk afnam. Bij 22% was dit meer dan 50% en bij 48% bleek de PSA vermindering tussen de 25%-50% te zijn. Van de 23 patienten ondergingen 15 mannen na 18 maanden weer een biopsie. Goedaardig weefsel werd bij 60% van de mannen gevonden. Vier van de 15 mannen (26.7%) hadden nog steeds HGPIN en bij 2 mannen (13.3%) werd prostaatkanker gediagnosticeerd. Vergeleken met een historische controle, had deze studiegroep goede resultaten. Na 18 maanden had maar 13.3% prostaatkanker, terwijl volgens de literatuur ongeveer 30% prostaatkanker na 6 maanden krijgt. Wel moet een duidelijke kanttekening gemaakt worden dat in deze studie niet alle 23 mannen een biopsie hebben ondergaan. Ook is het design van de studie niet geschikt om uitspraken te doen over de effectiviteit van de voedingssupplementen.

NF-κB, een eiwit dat het overschrijven in het DNA reguleert en een centrale rol speelt in de ontwikkeling van prostaatkanker, wordt als tumormarker gehanteerd bij tumorweefselonderzoek. In de biopten van de mannen die alleen Zyflamend hebben gebruikt bleek dat deze tumormarker significant was verlaagd in het weefsel ten opzichte van de andere groepen.

Tomaat / Lycopeen voor de prostaat! Lycopenen zijn plantaardige stoffen, die in groentes zitten, zoals bijvoorbeeld in wortels en vooral tomaten. Van die stoffen was al langer bekend dat ze biologisch actief zijn, en het vermoeden bestond dat ze misschien ook preventief konden zijn bij het ontstaan van kanker. In een kleine studie, die in het tijdschrift 'Journal of Nutrition' verscheen, in het Januari nummer van 2008, bleek dat lycopenen niet alleen meetbaar de prostaat vergrotring tegen ging, maar ook het PSA verlaagde. PSA is een voorspellende bloedparameter voor het ontstaan van prostaat kanker.

De studie bestond uit 2 groepen mannen die gerandomiseerd werden. Of placebo werd gegeven ????, of lycopenen in en dosis van 15 mg/dag voor 6 maanden. De resultaten waren opzienbarend:The primary endpoint of the study was the inhibition or reduction of increased serum prostate-specific antigen (PSA) levels. The 6-mo lycopene supplementation decreased PSA levels in men (P < 0.05), whereas there was no change in the placebo group.

The plasma lycopene concentration increased in the group taking lycopene (P < 0.0001) but other plasma carotenoids were not affected. Whereas progression of prostate enlargement occurred in the placebo group as assessed by trans-rectal ultrasonography (P < 0.05) and digital rectal examination (P < 0.01), the prostate did not enlarge in the lycopene group.Symptoms of the disease, as assessed via the International Prostate Symptom Score questionnaire, were improved in both groups with a significantly greater effect in men taking lycopene supplements. In conclusion, lycopene inhibited progression of BPH. (1)

Deze resultaten zijn niet nieuw, eerdere kleine studies wezen al diezelfde kant op. Maar ja, wetenschap is soms lastig, in een andere studie werd dit effect niet gevonden. Nu was die studie wel klein qua aantal patienten die onderzocht waren. Lycopenen zitten ook veel in tomaten, en studies naar de invloed van tomatensaus op bepaalde aspecten van prostaat kanker, liet ook positieve effecten zien.( 5, 6)

Dus al met al lijken lycopenen toch wel bruikbare stoffen, waarvoor het meeste bewijs ondersteunend is voor hun preventieve werking op het ontwikkelen van prostaat carcinoom, en mogelijk bestaat er ook therapeutische werking. Om dat definitief te bewijzen hebben we grotere studies nodig.

Urtica radix / Brandnetelwortels tegen prostaatklachten BPH

Brandnetels......dacht je dat je je daaraan alleen kan irriteren? Nee, het is ook zinnig als kruid bij moeilijk plassen door een lastige prostaat... Neem dan wel de wortel en niet de bladeren! Er zijn verschillende plantaardige middelen die bij klachten van goedaardige prostaathypertrofie ingezet kunnen worden. Brandnetelwortelpreparaten worden ook genoemd als zinvol in een grote studie met meer dan 600 patiënten die gedurende 6 maanden brandnetelpreparaat slikten. Na een half jaar bleek brandnetel significant de symptomen van goedaardige prostaat hypertrofie te verminderen. (1) Ook de objectieve maten zoals kracht van het plassen en de hoeveelheid urine die niet uitgeplast werd, veranderden alle in positieve zin. In een tweede studie, met wederom enkele honderden patiënten werd de combinatie van brandnetelwortel met Sabal (fruitextract van de zaagbladpalm) getoetst op werkzaamheid en veiligheid. Ook hier bleek dat het combinatiemiddel positieve en statistisch significante effecten had op symptomen van benigne prostaathypertrofie. De bijwerkingen waren mild tot afwezig.(2) In een Duitsel studie bleek de combinatie even effectief te zijn als een regulier middel (tamsulosine). (3) De combinatie brandnetelextract met Saw Palmetto extract bleek net zo effectief te zijn als het reguliere middel finasteride in een studie met 431 mannen met goedaardige prostaathypertrofie. De bijwerkingen waren echter alleen minder bij de kruiden. (4)

Een kwalitatieve bespreking (januari 2008) was sceptisch van toon, maar daar hadden de auteurs de eerste studie niet in de bespreking meegenomen.(5) Een andere bespreking noemde 4 studies en was positiever qua conclusie.

Beoordeling

De eerste studie demonstreerde duidelijk objectiveerbare en subjectieve positieve effecten van brandnetelwortel. De tweede studie alleen subjectieve effecten, objectieve effecten waren niet gemeten. Deze studie kwam uit Rusland. De kwaliteit van de studies is goed, maar er kan ruimte blijven voor enige twijfel. Vandaar een groen-oranje stoplicht voor brandnetelwortel bij BPH (benigne prostraat hypertrofie). Interacties en ernstige bijwerkingen werden niet gemeld.

Maca

Reducing prostate size in benign prostatic hypertrophy Both aqueous and hydroalcoholic extracts of red maca reduced prostate size but not yellow or black maca in a preclinical study. Interestingly, serum testosterone and oestradiol levels were not affected by any of the maca types assessed; however, red maca prevented an increase in prostate weight induced by testosterone treatment (Gonzales 2012). Previously, Gasco et al (2007b) suggested that red maca may inhibit dihydrotestosterone conversion by 5-alphareductase 2; however, this remains uncertain. A study investigating the mechanism further compared red maca to finasteride (a 5-alpha-reductase inhibitor), where it was shown to reduce prostate weight but not seminal vesicles, unlike finasteride, which reduced both. The study indicated a need for a direct measure of levels of reductase to clarify the mechanism of action (Gasco et al 2007b, Shrivastava & Gupta 2012).

BPH en pompoenpitten

Benigne prostaathyperplasie (BPH) is een goedaardige vergroting van de prostaat. Patiënten met BPH vertonen vaak lagere-urinewegsymptomen, wat tot uiting komt in moeilijk of pijnlijk urineren, frequent nachtelijk urineren en verminderde opslag van urine in de blaas. De gangbare medicatie kan een vermindering van de seksuele functie als bijwerking hebben. Om deze reden

hebben Damiano et al. de mogelijkheden van behandeling met de zaden van pompoen (Cucurbita pepo) onderzocht. De auteurs hebben een groot aantal (670) onderzoeken bestudeerd, waarvan er zestien in hun artikel zijn opgenomen.

Preklinische studies lieten zien dat extracten van pompoenpitten een antioxidatieve, anti-androgene en ontstekingswerende werking

hebben. Toediening van pompoenpitextracten verbeterde de blaasfunctie met betrekking tot een verhoogd blaasvolume en vermindering van de mictie (frequentie van urineren) en verminderde ook de groei van de prostaat. Daarnaast bespreken de auteurs zes klinische

studies, waaronder drie gerandomiseerde, dubbelblinde onderzoeken. De tijdsduur van de klinische onderzoeken varieerde van twaalf weken tot twaalf maanden. De zaden werden door de proefpersonen ingenomen als een ethanolextract (verhouding 15-25:1, ethanol 92%)

of als pompoenpittenolie. De studies wezen op een verbetering bij een pijnlijke en moeilijke urinelozing, toegenomen urineopslag in de

blaas, vermindering van het aantal nachtelijke urinelozingen en verbetering van de kwaliteit van leven. In een van de zes klinische studies

werd melding gemaakt van enige bijwerkingen: 4% van de proefpersonen kreeg milde gastro-intestinale klachten zoals diarree, misselijkheid en overgeven. In geen van de onderzoeken werd melding gemaakt van verminderde seksuele functie.

De auteurs concluderen dat een medische behandeling met pompoenpitten kan worden aanbevolen voor patiënten met mictiesymptomen gerelateerd aan BPH in een vroeg stadium, inclusief patiënten met symptomen van een overactieve blaas.

Saw Palmetto Shrinks Prostate Tissues

In the first American randomized clinical trial of saw palmetto (Serenoa repens [Bartr.] Small, Arecaceae), the respected prostate herb proved beneficial in reducing swelling of prostate tissues in patients with benign prostatic hyperplasia (BPH). While numerous studies have confirmed the ability of saw palmetto extract (SPE) to reduce BPH symptoms, this is the first evidence that it actually shrinks enlarged prostate tissues (Overmyer, 1999).

The randomized, double-blind, placebo-controlled clinical trial involved 44 men with symptomatic prostate enlargement who took 320 mg of saw palmetto extract with nettle root extract or placebo for six months (Nutrilite® Saw Palmetto with Nettle Root, Nutrilite Division of Amway, Ada, MI). Researchers tested the participants for the usual clinical parameters, including International Prostate Symptom Score (IPSS), maximum urinary flow rate, residual urine volume, prostate volume, and prostate specific antigen (PSA, described below). Unlike previous research, this study included an ultrasound-guided prostate biopsy, allowing the scientists to assess changes in the participants' prostate tissue. Biopsies were taken at baseline and after six months of treatment.

As with previous research, SPE proved effective (or "somewhat effective," according to the researchers) in reducing overall symptom score and maximum urinary flow. Although symptom score (IPSS) dropped in the SPE group, it dropped similarly in the placebo group (2.9 and 2.6 respectively). The urinary flow rate increased in the SPE group while declining in the placebo group. There were no effects on hormone levels or other blood parameters.

The most intriguing part of this story is that SPE suppressed swelling of the prostate epithelium, causing a contraction in the tissues in the epithelium of the prostate and the transitional zone. Interestingly, SPE produced this benefit without affecting the levels of testosterone or dihydrotestosterone, which means that SPE works by an unidentified but non-hormonal mechanism. This is especially good news for men because conventional drug therapy for BPH frequently causes hormonal side effects, including reduced sex drive and performance. It should also be good news for doctors, because SPE did not affect the level of PSA in the blood. PSA tests are used to detect prostate cancer, and conventional therapy interferes with PSA testing by masking this early warning sign.

An editorial by Leonard Marks, M.D. accompanying the research report should provide perspective for physicians still uncertain about herbal therapies. "Saw palmetto extract (SPE) should now be considered a treatment option for men with symptomatic BPH, absent complications of the disorder. SPE is extremely safe; it is likely to exert positive effects; many patients want it; and more potent remedies, i.e., drugs or surgery, are generally not required in most BPH cases." Marks points out that saw palmetto has been used in Europe for generations, that its safety "has never been seriously questioned," and with the exception of occasional stomach upset, no side effects have been reported. Protecting your prostate could be as easy as enjoying a delicious daily snack.

We already know that nuts have numerous benefits for the body -- they're a nutrient-packed powerhouse with plenty of protein and omega 3s.

And now new research shows that walnuts in particular may offer yet another benefit -- prostate protection. Researchers at UC Davis and the U.S. Department of Agriculture Western Regional Research Center in California found that a diet rich in walnuts could actually reduce the size and growth rate of prostate cancer.

There's a bonus, too -- such a diet could help ward off heart disease.

The lead researcher said, "Walnuts should be part of a prostate-healthy diet...that includes lots of fruits and vegetables."

People with prostate cancer have high levels of endothelin, a substance that increases inflammation of blood vessels and is a factor in heart disease. Walnuts reduce levels of endothelin as well as levels of LDL cholesterol.

In the study, researchers fed lab mice genetically programmed to develop prostate cancer the equivalent of 14 shelled walnuts per day for two months. The walnut-fed mice developed prostate cancers that were about 50% smaller than mice on a control diet. The cancers also grew 30% slower.

The walnut-fed mice also had lower levels of a particular protein the increases the risk of developing prostate cancer, and the walnuts had beneficial effects on multiple genes related to tumor growth.

The studies were in mice, but the researchers believe the same effects would be seen in humans. I know I'll be sending this good news along to every man I know.

Beweging voor een gezonde prostaat

Lichaamsbeweging is een efficiënt middel om kanker te voorkomen, niet in het minst prostaatkanker. Dat werd duidelijk na een onderzoekje bij mannen met verdacht prostaatweefsel. Mannen die een snelle tred hebben, blijken namelijk beter gevormde bloedvaten in de tumoren te hebben in vergelijking met mannen die een langzame tred hebben.

Beter gevormde bloedvaten staan gelijk aan een minder agressieve prostaattumor. Denk daarbij dat de meeste prostaattumoren niet behandeld hoeven te worden, omdat ze zich erg traag ontwikkelen en daarom niet levensbedreigend zijn.

Moet beweging deel uitmaken van een (prostaat)kankerbehandeling? Misschien wel, vindt professor Erin Van Blarigan (universiteit Californië). Hoewel de meeste onderzoeksresultaat dit bevestigen, zijn er echter gecontroleerde klinische studies nodig. Phytotherapy BPH

Phytotherapy is the use of plant extracts to treat a medical condition. Some plant extracts have been used for centuries to treat urinary symptoms secondary to BPH.

Today, there is wide variation in the use of these herbal products around the world. In some European countries, for example, Germany, Austria, and France, plant extracts are commonly used as initial treatment for men with urinary symptoms.

Saw palmetto (Serenoa repens)

Saw palmetto is one of the most researched herbal remedies for BPH. A three-year preliminary study in Germany found that 160mg of saw palmetto extract taken twice daily reduced night-time urination in 73% of patients, and improved urinary flow rates significantly.

Most recent studies have shown that to achieve the optimal benefits with saw palmetto, it is essential that fat-soluble saw palmetto extracts are standardised to contain 85% to 95% fatty acids.

Pygeum (Pygeum africanum)

Pygeum is the second most popular herb for helping with symptoms of an enlarged prostate. Pygeum extracts contain phytosterols that can inhibit prostaglandin synthesis, thereby reducing the abnormally high levels of prostaglandins normally found in BPH.

Pygeum has been approved in Germany, France, and Italy as a remedy for BPH. Controlled studies published over the past 25 years have shown that pygeum is safe and effective for men with BPH of mild or moderate severity.

Tomato (Lycoparsion esculentum)

Lycopene contains phytosterols that gives tomatoes their bright red colour. Lycopene gets high marks from researchers for its apparently potent antioxidant properties. Research studies show that high levels of lycopene consumption have been associated with long-term protection of prostate health.

Stinging nettle (Urtica dioca)

As men age, their sex hormone binding globulin (SHBG) level increases, which makes them more susceptible to abnormal cell proliferation. Stinging nettle has a strong affinity for SHBG, thereby limiting the amount of testosterone and oestrogen that can bind to it and influence cell proliferation.

This extract may increase urinary volume and the maximum flow rate of urine in men with early-stage BPH. It has been successfully combined with both saw palmetto and pygeum to treat BPH in double-blind trials.

Pumpkin seed (Curcubita pepo)

Pumpkin seed extracts contain phytosterols, curcubitin, and selenium. The extract inhibits 5-alpha-reductase, preventing the conversion of testosterone to dihydrotestosterone. It also has an inhibitory effect on aromatase, preventing androgens like testosterone from converting to oestrogen.

Selenium + silymarine bij plasproblemen

De meeste mannen krijgen vanaf een bepaalde leeftijd te maken met (goedaardige) prostaatvergroting. De combinatie van selenium en silymarine helpt de symptomen van prostaatvergroting terug te dringen, zo toont Tsjechisch onderzoek. Artsen gaven aan mannen met een prostaatvergroting 570 mg silymarine en 240 µg selenium gedurende zes maanden. De problemen bij het plassen verminderden: urineren ging beter en de PSA daalde. Er was geen effect op de testosteronspiegel.

Silymarine is het extract van mariadistel met silybine als voornaamste flavonoïde. In-vitrotesten tonen een remmende activiteit op de celdeling van prostaatcellen. De belangstelling voor selenium bij de preventie van prostaatkanker is altijd al groot geweest. Alle studies tonen dat een hoge seleniumconcentratie in het bloed garant staat voor een hogere bescherming tegen prostaatkanker. Een grootschalige trial ('SELECT') vond echter enkele jaren geleden geen bescherming.

Polyfenolen klinisch zinvol bij prostaatkanker

Een supplement rijk aan polyfenolen stuit de opmars van PSA bij prostaatkankerpatiënten. Patiënten die het supplement namen, kenden tijdens een periode van zes maanden een PSA-toename van 14 %, terwijl de gemiddelde toename in de controlegroep 78 % bedroeg. Bijna de helft van de patiënten uit de polyfenolgroep had een PSA lager of gelijk aan het oorspronkelijke niveau, gemeten aan het begin van de studie.

De deelnemers hadden een histologisch bevestigde prostaatkanker, maar werden er niet voor behandeld (active surveillance en/of watchful waiting). Een aantal patiënten hadden therapie achter de rug.

Het supplement bevatte extracten van vier 'superfoods':

  • granaatappel, 100 mg

  • broccolipoeder, 100 mg

  • kurkuma, 100 mg

  • groene thee, 20 mg, equivalent met 100 mg groene thee

Granaatappel is al in een gevorderde onderzoeksfase wat betreft prostaatkanker. Zelfs groene thee is al eens klinisch getest geweest. De preklinische studies rond broccoli en kurkuma zijn veelbelovend: kurkuma onderdrukt de symptomen van prostaatvergroting en het eten van broccoli geeft een gunstig genexpressieprofiel in prostaatkankercellen. De controlegroep nam een gelijkaardig supplement dat 10 mg waterkers bevatte, als kleurstof.

De artsen hadden bij de helft van de patiënten die onder active surveillance stonden, ook MRI-scans genomen. Bij mannen bij wie de kanker kromp, werd een tien keer lagere PSA gemeten. Voor de artsen is dit een kwalitatieve bevestiging van hun onderzoeksresultaten.

This article examines the role of the androgen dihydrotestosterone (DHT) in the healthy and diseased prostate and considers the implications of the data on DHT for therapeutic approaches to benign prostatic hyperplasia (BPH). Development and maintenance of the normal prostate, as well as development of BPH, depend on a functional androgen-signaling axis, components of which include: (1) testosterone synthesis in the testes and adrenal glands; (2) conversion of testosterone to DHT; (3) transport of DHT to target tissues; and (4) binding of DHT to the androgen receptor with consequent modulation of genes. DHT plays a beneficial role in the developing prostate but it can be detrimental in the adult prostate in that it causes pathologic prostate growth. The role of DHT in other adult tissues is uncertain. DHT has not been shown to perform beneficial functions unique from testosterone in the adult male, and it is believed that its fundamental effect is to amplify testosterone's weaker hormonal signal. Increased understanding of the cellular mechanisms by which the androgen-signaling axis functions has led to advances in treatment for prostate disease. In BPH, the 5alpha-reductase inhibitors--the only class of therapy to act at the pathophysiologic substrate of the disease--arrest the disease process, reduce prostate volume, improve symptoms, and reduce the risk of acute urinary retention and BPH-related surgery. The availability of dutasteride, the first dual (Type 1/Type 2) 5alpha-reductase inhibitor, offers the opportunity for rapid and consistent inhibition of DHT.

Benign prostatic hyperplasia (BPH) is one of the most common urological diseases in aging men. Because of its long latency, BPH is a good target for prevention. The aim of the study has been to review the various options of treatment, currently available, in the field of phytotherapy. Watchful waiting, pharmacological therapy, and surgery are also helpful, depending on the severity of the disease. Although drug therapy (alpha1-blockers, 5alpha-reductase inhibitors) and surgery (prostatectomy, transurethral resection, etc.) seem to be most effective for patients with moderate-severe BPH, herbal medicines (i.e., Serenoa repens, Pygeum africanum, Urtica dioica) are also commonly used in patients with mild-moderate symptoms. On the basis of preclinical studies several mechanisms of action have been postulated, including 5alpha-reductase inhibition, alpha-adrenergic antagonism, dihydrotestosterone and estrogen receptor inhibition. Randomized clinical trials indicate significant efficacy in improving urinary symptoms and mild adverse effects for some phytotherapeutic agents, while further clinical evidence is needed for others (e.g., Epilobium spp., Secale cereale, Roystonea regia). Healthcare professionals should be constantly informed about BPH phytotherapy, taking into account the risk/benefit profile of the use of medicinal plants in the management of BPH.

Benign prostatic hyperplasia (BPH), nonmalignant enlargement of the prostate, can lead to obstructive and irritative lower urinary tract symptoms (LUTS). The pharmacologic use of plants and herbs (phytotherapy) for the treatment of LUTS associated with BPH has been growing steadily. The extract of the African prune tree, Pygeum africanum, is one of the several phytotherapeutic agents available for the treatment of BPH.

OBJECTIVES:

To investigate the evidence whether extracts of Pygeum africanum (1) are more effective than placebo in the treatment of Benign Prostatic Hyperplasia (BPH), (2) are as effective as standard pharmacologic BPH treatments, and (3) have less side effects compared to standard BPH drugs.

SEARCH STRATEGY:

Trials were searched in computerized general and specialized databases (MEDLINE (1966-2000), EMBASE, Cochrane Library, Phytodok), by checking bibliographies, and by contacting relevant manufacturers and researchers.

SELECTION CRITERIA:

Trials were eligible if they (1) were randomized (2) included men with BPH (3) compared preparations of Pygeum africanum (alone or in combination) with placebo or other BPH medications (4) included clinical outcomes such as urologic symptom scales, symptoms, or urodynamic measurements. Eligibility was assessed by at least two independent observers.

DATA COLLECTION AND ANALYSIS:

Information on patients, interventions, and outcomes were extracted by at least two independent reviewers using a standard form. The main outcome measure for comparing the effectiveness of Pygeum africanum with placebo and standard BPH medications was the change in urologic symptoms scale scores. Secondary outcomes included change in urologic symptoms including nocturia and urodynamic measures (peak and mean urine flow, prostate size). The main outcome measure for adverse effects was the number of men reporting adverse effects.

MAIN RESULTS:

A total of 18 randomized controlled trials involving 1562 men met inclusion criteria and were analyzed. Only one of the studies reported a method of treatment allocation concealment, though 17 were double-blinded. There were no studies comparing Pygeum africanum to standard pharmacologic interventions such as alpha-adrenergic blockers or 5-alpha reductase inhibitors. The mean study duration was 64 days (range, 30-122 days). Many studies did not report results in a method that permitted meta-analysis. Compared to men receiving placebo, Pygeum africanum provided a moderately large improvement in the combined outcome of urologic symptoms and flow measures as assessed by an effect size defined by the difference of the mean change for each outcome divided by the pooled standard deviation for each outcome (-0.8 SD [95% confidence interval (CI), -1.4, -0.3 (n=6 studies)]). Men using Pygeum africanum were more than twice as likely to report an improvement in overall symptoms (RR=2.1, 95% CI = 1.4, 3.1). Nocturia was reduced by 19%, residual urine volume by 24% and peak urine flow was increased by 23%. Adverse effects due to Pygeum Africanum were mild and comparable to placebo. The overall dropout rate was 12% and was similar between Pygeum Africanum (13%), placebo (11%) and other controls (8%).

REVIEWER'S CONCLUSIONS:

A standardized preparation of Pygeum africanum may be a useful treatment option for men with lower urinary symptoms consistent with benign prostatic hyperplasia. However, the reviewed studies were small in size, were of short duration, used varied doses and preparations and rarely reported outcomes using standardized validated measures of efficacy. Additional placebo-controlled trials are needed as well as studies that compare Pygeum africanum to active controls that have been convincingly demonstrated to have beneficial effects on lower urinary tract symptoms related to BPH. These trials should be of sufficient size and duration to detect important differences in clinically relevant endpoints and use standardized urologic symptom scale scores.

Anti-androgens are an assorted group of drugs and compounds that reduce the levels or activity of androgen hormones within the human body. Disease states in which this is relevant include polycystic ovarian syndrome, hirsutism, acne, benign prostatic hyperplasia, and endocrine related cancers such as carcinoma of the prostate. We provide an overview and discussion of the use of anti-androgen medications in clinical practice and explore the increasing recognition of the benefits of plant-derived anti-androgens, for example, spearmint tea in the management of PCOS, for which some evidence about efficacy is beginning to emerge. Other agents covered include red reishi, which has been shown to reduce levels 5-alpha reductase, the enzyme that facilitates conversion of testosterone to dihydrotestosterone (DHT); licorice, which has phytoestrogen effects and reduces testosterone levels; Chinese peony, which promotes the aromatization of testosterone into estrogen; green tea, which contains epigallocatechins and also inhibits 5-alpha reductase, thereby reducing the conversion of normal testosterone into the more potent DHT; black cohosh, which has been shown to kill both androgenresponsive and non-responsive human prostate cancer cells; chaste tree, which has a reduces prolactin from the anterior pituitary; and saw palmetto extract, which is used as an anti-androgen although it shown no difference in comparison to placebo in clinical trials.

Lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH), a non-malignant enlargement of the prostate, are common in men older than 60 years worldwide. The availability of 5-alpha-reductase inhibitors, alpha-blockers, and phytomedicinal agents (herbal preparations) to treat the symptoms associated with this condition has reduced the need for surgery in these patients. However, the use of many of the conventional pharmaceutical drugs is associated with adverse effects, including postural hypotension (drop in blood pressure due to a change in body position, e.g., standing up) and sexual dysfunction. Thus, increasing attention has focused on the use of phytotherapeutic agents to ameliorate the symptoms associated with BPH.

The objective of this study was to assess the safety and efficacy of saw palmetto (Serenoa repens, Arecaceae) extract (Prostagood®; Dr. Willmar Schwabe GmbH & Co. Arzneimittel, Karlsruhe, Germany) and tamsulosin (Flomax®; Boehringer Ingelheim Pharmaceuticals, Ridgefield, CT), alone or in combination, for the treatment of LUTS secondary to BPH.

Sixty men aged 43–73 years with symptomatic BPH were enrolled between May and November 2005 in this 6-month prospective, open-label, randomized study, the location of which was not identified. The subjects were randomly assigned to 1 of 3 groups (n = 20 per group): 320 mg saw palmetto/day (referred to in this study as the SR group), 0.4 mg tamsulosin/day (TAM group), or 320 mg SR/day + 0.4 mg TAM/day (SR + TAM group). No placebo group was included. The International Prostate Symptom Score (I-PSS), the maximal urinary flow rate (Qmax), prostate volume, prostate-specific antigen (PSA) concentrations, qualityof-life scores, and postvoiding residual volume (PVR) were determined at baseline and at months 2, 4, and 6 of the study.

The I-PSS decreased significantly (P < 0.05) from baseline to 6 months by 6.1, 4.6, and 4.9 points in the SR, TAM, and SR + TAM groups, respectively. Qmax increased significantly (P < 0.05) from baseline to 6 months by 3.2, 3.7, and 4.3 mL/second in the SR, TAM, and SR + TAM groups, respectively; however, the differences between groups were not significant. Prostate volume, PSA concentrations, quality-of-life scores, and PVR decreased in all 3 groups by 6 months, but not significantly in any of the groups. No adverse effects were observed in the SR group, and those that were observed in the 2 other groups (e.g., postural hypotension, dizziness, decrease in libido, dry mouth, and fatigue) were not serious enough to require withdrawal from the study.

Treatment of the urinary symptoms of BPH with saw palmetto extract or tamsulosin alone was equally effective, while treatment with a combination of the herbal extract and the drug conferred no extra benefit. The authors conclude that saw palmetto extract “is a well-tolerated agent that can be used alternatively in the treatment of LUTS due to BPH.”

Gember remt prostaatkanker

Bijna alle mannen krijgen, als ze maar oud genoeg worden, prostaatkanker. Als je in de prijzen valt openbaart de ziekte zich op je zestigste en moet je de rest van je leven aan de anti-androgenen. Niet leuk. Vandaar dat de groeiende interesse in goedkope en niet-toxische supplementen die prostaatkanker afremmen. Onderzoekers van Georgia State University hebben een nieuwe gevonden: gember-extract.

Mannen zijn soms pechvogels. In hun seksuele organen zit een tikkende tijdbom - de prostaat. Naarmate het ding met het klimmen van de jaren groter wordt, groeit de kans dat celletjes in het orgaan veranderen in kankercellen. Mannen met een hoge natuurlijke testosteronspiegel hebben in epidemiologische studies minder kans op prostaatkanker, maar bij oudere mannen is het precies andersom.

Er zijn aanwijzingen dat mannen die veel synthetisch foliumzuur en zink binnenkrijgen meer kans op prostaatkanker hebben. Daarom krijgen mannen die veel zwaar gedoseerde multivitamines slikken vaker prostaatkanker. Aan de andere kant vertellen studies dat vette vis, soja, magnesium en uiteraard lycopeen in tomatenproducten mannen kunnen beschermen tegen kanker. Voeg je daar de dierstudies nog bij, dan zijn ook curcumin, een koolhydraatarm dieet en broccoli interessant voor mannen die hun kans op prostaatkanker willen verminderen.

In vitro-studie

In de British Journal of Nutrition verschijnt binnenkort een Amerikaanse studie waarin een extract van gember alias Zingiber officinale menselijke prostaatkankercellen doodt.

De onderzoekers deden proeven met cellen in reageerbuizen, en zagen dat die vaker stierven toen de concentratie in de buurt van de 100 microgram/milliliter kwam. Gezonde prostaatcellen [PrEC] en humane primaire fibroblasten [HDF] stierven niet. GE = gember-extract.

Dierstudie

De onderzoekers implanteerden menselijke PC-3-prostaatkankercellen in muizen. Een deel van de muizen kreeg elke dag oraal 100 mg per kg lichaamsgewicht gember-extract toegediend, een ander deel niet. In de 8 weken dat het experiment duurde halveerde het extract de groei van de tumor. Het humane equivalent van die dosis is - als je uitgaat van een man van 70 kg - 567 mg extract per dag. Dat kun je halen uit een ons verse gember, schatten de onderzoekers.

Het extract remde de aanmaak van Cyclin B1, D1 en E. Cyclines zijn eiwitten die de ontwikkeling van een cel aansturen. Bij kankercellen is de aanmaak van die eiwitten verhoogd. Het extract verhoogde de concentratie p21 en actief Caspase-3. Die laatste twee eiwitten zijn actief als cellen zelfmoord plegen. Kankercellen hebben hun zelfmoordmechanismen uitgeschakeld, maar gember schakelt het kennelijk weer in.

Conclusie
