Angelica sinensis / Dong quai

Dong Quai wordt al honderden jaren gebruikt in Traditionele Chinese, Koreaanse en Japanse geneeskunde. Het wordt voornamelijk gebruikt door vrouwen, het wordt dan ook 'vrouwen ginseng' genoemd: vanwege het gebruik bij gynaecologische afwijkingen en herstel van een bevalling of ziekte (30). In de Chinese geneeskunde wordt de plant in traditionele formules benut voor het voeden en versterken van het bloed, de stimulering van de doorbloeding en het verzorgt de regulatie van de menstruatie cyclus (33, 34).

Indicaties

Menopauze

Er zijn klinische aanwijzingen dat Dong Quai overgangsklachten zoals opvliegers en overmatig zweten verbetert bij gebruik in combinatie met andere bestanddelen zoals OPC en fyto-oestrogenen Hop en Rode klaver in vergelijking met een placebo (18). Ook met betrekking tot andere combinaties met Magnesiumtauraat wordt er verbetering gezien bij de kwaliteit van de slaap. De symptomen verminderen (18, 31, 32, 36).

Migraine

Onderzoek toont aan dat het nemen van een combinatie van fyto-oestrogenen en Dong Quai gedurende 24 weken, de frequentie van hormonaal gerelateerde migraine aanvallen verminderen in vergelijking met een placebo (19).

Zwangerschap gerelateerde complicaties

Voorlopig klinisch bewijs suggereert dat het nemen van een TCM formule met Dong Quai, als moederkruid in combinatie met witte pioen en Ligustra de kans op perinatale mortaliteit vermindert gedurende de hele zwangerschap vergeleken met een controlegroep (20).

Pulmonale hypertensie

Sommig voorlopig klinische onderzoek suggereert dat intraveneuze toediening van 250 ml oplossing met dong quai 25% eenmaal daags gedurende 5-10 dagen zowel pulmonale arteriële druk als de pulmonale vasculaire weerstand bij patiënten met COPD vermindert. Bovendien verbeterde de pulmonale hypertensie (21, 22, 23). Meer onderzoek is nog nodig om Dong quai voor deze toepassingen te bewijzen.

Veiligheid

Dong quai is veilig gebruikt in een klinisch onderzoek dat tot 24 weken heeft geduurt (9). Meer bewijs is nodig om de veiligheid na langdurig, herhaald gebruik te bepalen. Tijdens zwangerschap zou het theoretisch een negatief effect hebben aangezien het de baarmoedercontracties kan stimuleren vooral in combinatie met peterselie. Tijdens de eerste trimester van de zwangerschap wordt het afgeraden te gebruiken (24). Voor borstvoeding is er onvoldoende onderzoek om de veiligheid vast te stellen.

Interacties met kruiden

Het gelijktijdig innemen van kruiden die antistolling effecten nastreven kan theoretisch het risico van bloedingen verhogen wegens de synergie (25). Dit zijn kruiden zoals ginkgo, kruidnagel, knoflook, gember, ginseng, populier, wilg en anderen.

Interacties met medicijnen

Het gebruik van Dong Quai heeft aangetoond bloed voedende en verdunnende kwaliteiten te bezitten. Het gelijktijdig innemen van kruiden die antistolling effecten nastreven kan theoretisch het risico van bloedingen verhogen wegens de synergie (26). Dit zijn medicijnen zoals aspirine, Plavix, dalteparine (Fragmin), enoxaparine (Lovenox), heparine, ticlopidine (Ticlid), wafarine (Coumadin) en anderen (25, 26, 27). Dong Quai heeft oestrogene effecten (10, 11, 13, 28, 29). Theoretisch bestaat de kans dat bij inname van grote hoeveelheden Dong Quai kan interfereren met hormonale substitutietherapie, als gevolg van concurrentie voor oestrogeenreceptoren.

Bijwerkingen

Dong Quai wordt goed verdragen (9). Zeer zelden is melding gemaakt van bijwerkingen bij gebruik van de plant of plantenextract.

Contra-indicaties

Vrouwen met een voorgeschiedenis van borstkanker, eierstokkanker, endometriosis wordt afgeraden Dong Quai te gebruiken gezien de oestrogene werking (10, 11, 13, 28).

Dosering

Voor de menopauze kan een dosering van 100 mg van het gestandaardiseerde extract voldoen mits het wordt gecombineerd met OPC, Magnesium en fyto-oestrogene kruiden zoals Hop en Rode Klaver. Bij het enkelvoudige kruid wordt dagelijks een 5 gram wortelpoeder gebruikt (9, 31).
