Overview of Review Session:
Before the session starts: Complete debrief form and Leadership Quiz (handouts)
1. Introduction
2. Team case presentations from Visit # 2:
- 3 - 4 minutes per team presentation
- Share education tool information
- Describe results from:
- Stay Independent and Home Safety checklists,
- Rapid Geriatric Assessment
- Get Up and Go
- OHAT or OPE
- Other assessments completed
- Opportunity for questions and suggestions from other teams/participants.
3. Facilitated review of materials and team preparation for Visit 3:
- Re-establish rapport with team and patient.
- Update health history with any changes since the last visit; include current medications and the reason they're prescribed.
- Assess patient vital signs/physical exam noting any changes.
- Review last visit with your patient.
- Ask about the impact of stress/difficulties (including perceived discrimination, if appropriate) on health and well-being.
- With the patient, work through the:
- Two-Question Depression Screen
- Duke Social Support Survey
- Ask the patient about goals/plans to maximize his/her health and wellness.
- Use Brief Motivational Interviewing to identify goals/patient's unique resources/challenges
- Collaborate with the patient to make Action Plan
- Review/Describe community resources available for changing behaviors
- Discuss with the patient how team/team members (or health care providers in general) might improve their interactions with patients.
- Thank your patient for being an integral part of your education.
4. Teamwork assessment and discussion:
- Discuss/identify methods for improving performance in the team in a small group
- Share team strengths/areas for further work with the large group
5. Collect teamwork debrief forms, PINK copy of patient report form & RGA Form
