Preparing for Interprofessional Working

Objectives:

  1. Participate in a mutually respectful, non-hierarchical environment for first team meeting
  2. Describe expectations for the Health Partners program
  3. Describe common goals for all teams/team members
  4. Experience positive IP social interaction including discussion of language demonstrating IP "cultural competence."
    • What stereotypes do we have about each other?
    • How are we the same? Our common skills and knowledge?
    • How are we different? Our unique skills and knowledge?
    • When are hierarchies helpful/dangerous?
    • What attitudes and behaviors help us work together well? How does language impact working relationships?
    • What are the unique teamwork challenges of clinic based care vs. hospital care?
    • How do we decide who gets respect, how much, and why?
    • Ownership language
  5. Describe core competencies for Interprofessional Collaborative Practice as promulgated by IPEC (the Interprofessional Education Collaborative), May 2011/2016, to include:
    • Values / Ethics
    • Roles / Responsibilities
    • Interprofessional Communication
    • Teams and Teamwork

Activities: 5:15 pm -

  1. Pick up meal and find your team.
  2. Complete required forms, share in team discussion.

5:30 pm -

  6. Facilitated review of materials and team preparation for Visit 1
    • Objectives and Activities for Visit 1
      • Assessment tools located under "Visit 1" on website.
        • RGA: First three sub-scales + Advance Directive question
        • Stay Independent Checklist
        • Home Safety Checklist (leave as homework assignment)
      • As appropriate/indicated, administer optional screening tools:
        • Nutrition Screen
        • Katz ADL or Lawton-Brody IADL Scale
        • 3 question Health Literacy Screen / Newest Vital Sign Tool
    • Team huddle to determine:
      • Which team member does what to arrange visit #1 and who leads on which V-1 tool
      • 5 minutes for tool review; brief other team members
  7. Complete and turn in evaluation and other forms as directed.
