WHAT TO BRING

Please bring the following:

1. A GP or Specialist referral. Please note this practice does not accept indefinite referrals for 'skin checks'.

2. Completed "Patient Information" form. Click on the link on the left to download/ print the pdf.

3. Medicare Card, Health Care Card, Pension Card.

4. A list of all medication you are taking, including the names of any creams you are using.

FEES

Please note that we are a private clinic and full payment is required on the day.

Fees are discounted for Health Care Card holders, Aged Pension Card holders, Disability Pension Card holders, and DVA Card Holders.

The "Seniors" card is not accepted.

EFTPOS, Mastercard, Visa Card are accepted.

AMEX is not accepted.

APPOINTMENTS

Please ring 9228-0795 to cancel your appointment if you are unable to attend. We require 48hrs notice for cancellations.

Please call 9228-0795 to let us know AS SOON AS POSSIBLE if you cannot make a surgical/biopsy appointment.

SURGERY

Most surgery will involve sending the skin sample for 'pathology'. We use only specialist skin pathologists. We have a special arrangement with our pathology provider. There will be no out-of-pocket fee for your pathology. At other dermatology practices in Perth, you will be charged an additional pathology fee which can be as much as a few hundred dollars per biopsy.

Surgery is almost never done on the same day as your consultation appointment. In this way, we can run on time.

Unlike most other practices we aim to give you an accurate estimate of cost for all surgical procedures.

URGENT APPOINTMENTS

If you feel your condition is urgent, please ask your GP to contact the practice to discuss.

We often have some urgent apppointments available each week- however these can only be accessed by request from your GP.

As you can imagine most people think that they have an urgent problem! We always prioritise patients who may have serious problems like serious or rapidly growing skin cancers or urgent rashes.
