Thank you for your interest in the wonderful world of therapy dogs!

Once you and your dog have passed the AKC Canine Good Citizen (CGC) test and passed our evaluation you are eligible to register for membership and visit through ITD, Inc.   All the forms necessary to file your Application Packet are download from this page.  Application packets must be received within 30 days of your evaluation date.

Each of our forms was designed to fit on a single page.  You may need to adjust your printer margins to print each form on one page.

A complete application packet includes:

• • • Membership Application  (form must have an original applicant/member signature - no copies or faxes.)

    • Rules & Regulations  (form must have an original applicant/member signature - no copies or faxes.)

    • A copy of your ITD, Inc. evaluation form (signature of Evaluator must be legible.)

    • A copy of your AKC CGC certificate or a copy of the test form signed by CGC tester

    • Annual Health & Vaccination Record  (form must have an original Veterinarian's signature)

    • Photo Release  (form must have an original applicant/member signature - no copies or faxes.)

    • Check or money order for applicable fees

    • 2 different emailed jpeg. format dog head shots (must have a light, solid color background)
