Health Maintenance

Well Child Visits:

- 2 Weeks Visit

- 2 Month Visit & Immunizations

- 4 Month Visit & Immunizations

- 6 Month Visit & Immunizations

- 9 Month Visit & Developmental Screen

- 12 Month Visit, Immunizations, Lead Screen #1 & HCT

- 15 Month Visit & Immunizations

- 18 Month Visit & Immunizations

- 2 Years Visit & Lead Screen #2

- 3-4 Years Visits & Annual Physical Exams

- 5 Years Visit & Kindergarten Physical

- 6-9 Years Annual Physical Exams

- 10-19 Years Annual Physical Exams & Immunizations

Male Exams:

- Age 18+ requires a yearly physical

- Colonoscopy after age 50 (earlier if family history) and up to age 75, repeated every 10 years, or more frequently based on results.

- PSA (Prostate cancer screening) yearly after age 50

Female Exams:

- Over the age of 18 Annual Physical

- Pap smear every year or as indicated:may skip 1 year after 3 consecutive normal's

- No pap smear after age 65

- Colonoscopy after age 50 (earlier if family history) and up to 75, repeated every 10 years, or more frequently based on results

- Mammogram yearly after age 40 Baseline between 35 &40 years

- Total hysterectomy for reasons other than cancer, no pap smear needed

- Total hysterectomy for diagnoses of cancer, pap smear every 5 years

-Bone Density after age 50, every 2 years

Physician Services

Treatments:

- Major/Minor Joint Injections

- Skin tag removal

- Lesion/Mole removal

- Wart treatment/removal

- Punch Biopsies

Physical Examinations:

- DOT Physicals. ( Only by Lauralee Sibiga, PA-C) DOT physical's ARE NOT billed to your insurance, they are $200 upfront at time of visit (cash or credit only), and DO NOT count as your yearly physical.

- College/School Physicals.

- Pre-Surgical Clearance History & Physical.

All standards are left open to the discretion of the provider. Chronic illnesses such as DIABETES, HYPERTENSION, ASTHMA, HEART DISEASE, and LUNG DISEASE should be followed up every 3-4 months or as indicated by your provider. You have the right to refuse physicals and preventative tests, but you will be asked to sign an INFORMED CONSENT. Please realize that these preventative procedures are in the best interest of your health care.

Diabetic Patients:

- Follow up appointments for diabetes should be every 3 months (4 times yearly) this includes an annual diabetic foot exam

- Eye exams annually with an Ophthalmologist

- Blood work:

Hemaglobin A1C: Every 6 months (blood sugar)

Microalbumin: Once a year (kidney function)

LDL: Once a year ( cholesterol)

GFR: Once a year ( kidney function)
