Family ID

All Parents and Students must register on FAMILYID to be eligible to participate/play

HOW TO REGISTER

  1. Go to www.familyid.com
  2. Create a log in.
  3. Select "I Agree to the Family ID Terms of Service".
  4. Click Create Account.
  5. You will receive an email with a link to activate your new account.
  6. Click the link in the activation email.
  7. Click on "Get Started Now".
  8. Click on Find a Program.
  9. The Program is Hyde Park Central School District.
  10. Click on Modified Sports for the spring season.
  11. Click Register Now.
  12. Complete the registration. Upload the most recent physical and doctor's orders for any medications if needed.
  13. The student MUST electronically sign under the Parent Athlete Concussion Agreement and the HPCSD Handbook for Student Athletes and Parents where indicated. The parent signs each section.
  14. Once all sections have been completed, click submit.
  15. The nurse will review the registration and send a follow up email if any items are missing or require clarification.
