New Student Requirements:
- Copy of birth certificate
- Copy of Social Security card
- Up-to-date immunization record (including the Tdap booster before student can enter 7th grade)
- Current physical address and telephone number(s)
- Parent/guardian information (custody papers if applicable)
- 504/IEP plans if applicable
- Transfer school information to include attendance, discipline, transcript, and testing
If you'd like to have a conference with your child's teacher(s), please call me at 346-1011 or email to: wanda.parsons@lee.k12.va.us so we can setup a meeting time during the teachers' planning block or before/after school. Phone conferences or virtual meetings may be preferred due to visitor restrictions and social distancing requirements.

1 elective
At this time, we are offering the following electives to choose from
for 2024-2025 school year: Art 6, Band 6,and Computer
Science/Keyboarding. Please list your 1st and 2nd choices below:

JMS Upcoming 7th Grade Course Form
***Please note, the Virginia Department of Education requires all
upcoming 7th grade students to receive the Tdap vaccine before
beginning the 7th grade school year. Please have your child’s
immunization record faxed to JMS (346-1411) or you can scan/email the
immunization record to: wanda.parsons@lee.k12.va.us
