Mail copy of registration and check payment $110 by June 30th, Late registration $150 after June 30th  

US Lacrosse Membership is required for registration, this is used for insurance purposes.  US Lacrosse membership is good for one year for any future events, and can be obtained at www.usalacrosse.com.  So many benefits and lacrosse information provided with membership including annual subscription to US Lacrosse Magazine.

You will be asked to mail a check for $110 postmarked no later then June 30th along with the responses to your registration.  Please SIGN your waiver response!!  

Late registration after June 30th requires additional fee and will be collected by cash/check first day of camp.  $150 along with completed registration -print responses and sign by waiver and bring with check
