Published by Macmillan in 1993, it was limited to 430 numbered copies of which 130 included a portfolio containing separate prints of the nine wood-engravings by Holiday.

This early French edition was published by GLM Paris in 1948 and was limited to1080 numbered copies. It was reprinted in 1970 by Le Club Francais du Livre in a limited edition for their members.
