How to Register for Online Services
To use the Online Services, each patient requires a USERNAME and a PASSWORD. These are issued by our receptionists (by hand only). Before asking for the user name and password, you will need to complete the following form (print and sign). Then, visit our surgery and hand this form in with a proof of photo identification e.g. passport/driving license/etc.
