Athletic Physical Forms for Participation in an Athletic Sport

You will need the PPE form (see link below) to be completed annually by your physician and reviewed by the school doctor:

Clearance to Return to Athletics Following COVID-19 Form (COVID-19 Clearance) **As per the most recent guidance from our school physician, we are no longer requiring this form to be completed for mild/asymptomatic illnesses (as of 11/14/2022). Please note, that this form may be required in certain circumstances of known moderate/severe illness.**

Important Physical Examination Information

Athletic physicals are good for 365 days. If the athletic physical is due to expire during a current season, the physical is still considered "current" for the remainder of that season.  A new physical will be required before the start of the NEXT season.

  

 

You are encouraged to have your personal physician complete the physical paperwork when the student-athlete has their annual physical. Because the AFC Urgent Care in New Providence is our school physician, all Sports Physical paperwork must be taken to their office for final approval. Please take this completed packet to the AFC at 1286 Springfield Avenue, New Providence, NJ (908-402-3338). After reviewing the submitted PPE, the school physician will sign and stamp it in the appropriate location(s) and return the packet in its entirety to you via email. You may then submit it to the Athletic Office (as a PDF Document*) via step 4 of the digital rSchool Athletics Registration portal.

 *ALL FORMS SHOULD BE SUBMITTED TO AFC URGENT CARE AND THEN UPLOADED TO THE rSCHOOL REGISTRATION SITE AS A PDF DOCUMENT. If you do not have a document scanner, there are many free apps for smart phones and tablets that can use your camera to convert images to a batch PDF file. One such app is called, "Cam Scanner". Please be sure that the entire packet is uploaded as one file. This is called a "batch scan."*
