Instructions for Sports Physicals

In order to be medically eligible to participate in athletics, an athlete will need:

Upon successful completion of all of the above and a review by the New Providence Athletic Department, parents/guardians will receive an email confirming that their son/daughter is now eligible to participate in athletics.


FURTHER DETAILS ON PPE PROCESS:

Any student participating in an athletic activity MUST have a sports physical (PPE) completed prior to the first practice session. Athletic physicals are valid for 365 days. If your physical exam was conducted more than 90 days prior to the first practice session, a health history update form must be filled out. The PPE can be obtained via links below or links in the rSchool Athletics Registration portal. If any injury or illness did occur after the original physical was done, further medical clearance is required. 

In accordance with the New Jersey State guidelines for athletic physicals, “Each student's medical examination shall be conducted at the medical home of the student (a healthcare provider chosen by the student's parent/guardian for the provision of healthcare). If a student does not have a medical home, the district shall provide this examination at an appropriately equipped facility. For the purpose of the sports physical examination only, students' parents may choose either the school physician or their own private physician. It is preferable to have the examination conducted by your private physician since he/she already has a baseline medical history of your child as well as a current immunization record. Additionally, should you choose the school physician to complete the physical, the fee will be $50 (charged by AFC URGENT CARE in New Providence).

Once your primary care physician has completed the physical exam paperwork, please take this entire packet to AFC Urgent Care, located at: 1286 Springfield Avenue, New Providence, NJ (908-402-3338) - where the school physician will review them. 

The school physician will grant medical clearance by stamping the physical examination form. He/she will also provide you with a signed/stamped letter stating whether your child can or cannot participate in the sport. 

Upon receiving the PPE and signed approval letter from AFC Urgent Care, the entire document should then be uploaded (as a PDF document*) to the rSchool Activity Registration Portal (when prompted on Step 4). 

*ALL FORMS SHOULD BE SUBMITTED TO AFC URGENT CARE AND THEN UPLOADED TO THE rSCHOOL REGISTRATION WEBSITE AS A PDF DOCUMENT. If you do not have a document scanner, there are many free apps for smart phones and tablets that can use your camera to convert images to a batch PDF file. One such app is called, "Cam Scanner". Please be sure that the entire packet is uploaded as one file. This is called a "batch scan."*

**All Health History Updates, Policy forms (concussion, sudden cardiac death, steroid testing, opioid use/misuse, etc.), Emergency Contact Information, and Athletic Permissions must all be completed ONLINE.**

As stated above, physical exam paperwork and the corresponding AFC cover letter WILL BE SUBMITTED DIGITALLY during the rSchool registration on an annual basis.


PLEASE REMEMBER: NO STUDENT MAY PARTICIPATE (PRACTICE OR PLAY) IN ANY SPORT UNTIL THESE FORMS ARE COMPLETED AND YOU RECEIVE A CONFIRMATION EMAIL FROM MRS.
