Please submit a well-lit, recent passport-style photo meeting the following requirements:

● The photo should be a color image

● You must be facing forward, with your head and shoulders clearly visible

● No photos with sunglasses or hats please

● We can crop in to your photo but make sure you are the only one in the picture

● A phone "seflie" is fine as long as it meets the above requirements!
